Registration process and user delay

Hello, I just noticed a huge problem, on all apps, not just this one. I will try to be as clear as possible.
In the registration process, we have drop-down menu fields. For example here we have “Establishments”.
When creating this app, we had to put the word "Choose" as the first choice, so that people would not leave "Brest", which was displayed originally. The process having changed, it no longer displays the first choice. Yesterday I removed “Choose”.
Which actually shifts all the old choices in the user profiles. For example, Jeannot Hervé who was part of "Ollioules", finds himself with the establishment "Paris/Bagneux" in his profile.
And it's the same for all users.
It's also the same if I add a new city among the others in the registration process, it shifts everything in the profiles.
So I was forced to recreate a first choice where I only put one point, so that it would put the right choices back into the profiles.
This problem absolutely must be remedied and a change in the registration process does not disrupt everything in the profiles, otherwise there is nothing we can do.
